DEAF BLIND WITH MULTIPLE DISABILITIES (DMBD) CASE MANAGER
Euless, TX

The DMBD Case Manager position is available in the Intellectual and Developmental Disabilities program of VOATX that supports in-home support, therapies, and day habilitation.  This position coordinates and supervises the delivery of all DBMD Waiver supports which includes determining eligibility, developing support plans and goals, coordinating supports, and monitoring services. 
 
Responsibilities Include:
  • Organizes and directs services to ensure compliance with applicable federal law, state certification principles for participation in the Deaf, Blind and Multiple Disabilities program, and VOATX policies.
  • Responsible for the development of each participant’s Implementation Plan related to their Individual Program Plan (IPP).
  • Informs individual and/or his/her legally authorized representative about recipient’s IPP, any agreed-upon changes in IPP prior to implementing the changes, and is available to answer their questions about the IPP.
  • Ensures that documentation of all supports delivered to participants is accurate and timely in accordance with VOATX policies and the applicable DBMD billing guidelines.
  • Ensures appropriate Level of Need assignments and initiates requests for changes to DADS.
  • Ensures all state required documentation including IDIRCs, IPPs, ICAPs, quarterly reviews, etc. are completed by required deadlines; to ensure continuity of care.
  • Records and maintains documents in a timely manner: completes staffing reports, IPCs, IPPs, ICAPs, training documentation, case notes, and other required documentation.
  • Supervises and coordinates transportation as needed.
  • Coordinates the scheduling of participants’ programs and/or care plan needs. 
  • Ensures the health and welfare of all participants.
  • Coordinates medical needs with Nursing.
Qualifications:
  • Bachelor’s degree in social sciences or related field plus one year experience with intellectual and developmental disabilities, OR a high school diploma/GED plus five years direct experience with intellectual and developmental disabilities, OR the equivalent combination of education and experience.
  • Social work certification and supervisory experience are highly preferred. 
  • Competency in ASL preferred.
  • Ability and means to travel on a flexible schedule as needed.
  • Flexibility of scheduling work hours to meet the needs of the program.
  • Skill in the use of a personal computer and software applications (e.g. Microsoft, Word, Excel,  PowerPoint, Electronic Health Record, etc.).
  • Strong organizational skills and the ability to maintain detailed records.
  • Displays passion and optimism; inspires respect and trust, and treats people with respect.
  • Ability to interact with staff members and community contacts.
  • Demonstrates ability to work with multi-disciplinary team.
  • Must be able to work independently with little supervision.
  • Ability to exercise initiative and sound judgment and to react with discretion under varying conditions.
  • Must have valid Texas Driver License (Must be received within 30 days of employment) and ability to be meet company insurance requirements (with satisfactory MVR record); valid Vehicle Liability if driving personal vehicle.
